Any changes made in ownership, address, phone numbers, licensed supervisory personnel, or name of company must be sent to the City of Tualatin Building Division at building@tualatin.gov in writing, immediately. The undersigned hereby certifies that the information contained on this license application, together with attachments, is accurate and true. The undersigned further agrees that, if granted a license to operate in the City of Tualatin, the applicant will comply with all City of Tualatin building, plumbing, mechanical, electrical, Tualatin Municipal, Tualatin Development, and ancillary codes and amendments as adopted by the City of Tualatin and will comply with the license regulations and responsibilities of the City of Tualatin and the State of Oregon.
